H4 Hospital - Housing and Health support

This programme links 1-1 Housing and Health support with patients at in Stockport and Wythenshawe who are homeless or at risk of homelessness. We work with the hospital teams to reduce unsafe and unplanned homeless discharges and repeated use of A&E services. We have two dedicated members of staff working between Stockport Homes' temporary accommodation schemes and acute services to ensure that homeless people are supported to access appropriate resources and accommodation, and reduce the number of admissions and reduce unplanned discharges.

The project also provides holistic follow up support to all patients to help them manage their health and housing needs better in the community. This includes ensuring GP sign ups, facilitating access to services such as drug and alcohol, mental health and homefinder support, introduction to in-house volunteering opportunities, and peer support mentoring.
