Personalised Care and Support Planning Tool

Here's the shared example

Think Local Act Personal (TLAP) were commissioned by the Programme Management Office (made up of the Department of Health, ADASS and the LGA) to develop a personalised care and support planning tool. The tool aims to support councils and other people and groups to put the Care Act into practice.

This tool is aimed at leaders, commissioners, planners, clinicians and practitioners involved in designing and delivering personalised care and support planning for people with a variety of health and social care needs.

Through a series of case study scenarios, developed with people in the field - clinicians, social care managers, voluntary sector partners and people with lived experience of care, the resource demonstrates what different journeys through personalised care and support planning could look like when delivered through integrated and person-centred arrangements. It also begins to unpack many of the workforce, commissioning and organisational implications.

The characters developed for this tool have physical, mental and social care issues and are eligible for personal budgets for health and/or social care. Their journeys through the system and their interaction with different services, clinicians and professionals are designed to demonstrate how people with complex health and social care needs can experience a joined up approach to personalised care and support planning that improves health and wellbeing outcomes.
