Three Cs stands for ‘Control and Choice in the Community’ and is a progressive provider of services for people with learning disabilities and/or mental health challenges. Three Cs aim to support people to have control over and choice about their lives and to contribute to the community as equal citizens.
Three Cs support about 300 people, are based in Greenwich, London and support people in the boroughs of Lewisham, Southwark, Newham, Redbridge and Greenwich. Three Cs do this through Community Support, Supported Living, and Social Inclusion and Day Services.
Three Cs wanted to measurably increase quality, completion rate, accountability and delivery of outcomes for the people they supported.
Three Cs worked with Aspirico to develop a service for their care and support planning. The iplanit suite is an internet based service for providers who are focused on evidencing value and person centred service delivery. iplanit is used extensively by providers supporting people with learning and other disabilities, and the service can be tailored to support people with learning disability, mental health issues, older people, people with physical disability and those in supported living services.
During the project the team put significant emphasis on the following key principles when communicating the plan to stakeholders:
- Securing accountability for achievement of all person-centred outcomes, including more challenging goals
- Celebrating success in person-centred outcomes based on recorded and measurable evidence
- Streamlining cumbersome paper based processes to improve cost-effective delivery of services and making savings in administration and reporting overhead.
- Making it happen – Empowering people by putting the action and outcomes into planning
The programme ensures everyone supported by Three Cs has a user-friendly account accessible online via a computer or a handheld device where outcomes and actions agreed in person-centred planning are logged. This helps make sure that the outcomes most important to the person are kept in view until they have been achieved.
Using the iplanit computer programme also ensures that progress and service quality across the organisation can be tracked and monitored centrally. After implementing iplanit, Three Cs monitoring found that people in paid employment, education and voluntary work had increased, showing that the organisation and planning programme helped people to achieve their goals. Three Cs can also use iplanit reports to evidence values to commissioners.
