I Statements: defining the goals of integrated care

A narrative for person-centred coordinated care

 

This narrative describes what good quality integrated care looks like, from the point of view of anyone who needs access to multiple services over time. Created with the help of Think Local Act Personal (TLAP), and working with service users, carers, charities, commissioners and professionals, it was adopted as a national definition for integrated care by all the system leading bodies in England, including the Department of Health in a national 'Shared commitment' document.

No assumptions

This is the second of National Voices' narratives on person-centred coordinated care. This narrative has been jointly published by National Voices and Think Local Act Personal, developed in partnership with NHS England through its voluntary sector Strategic Partners Programme. It describes some critical outcomes and success factors in the care, support and treatment of people who use mental health services, from their perspective.

This is the fifth of National Voices' narratives on person- centred coordinated care. In this narrative, National Voices and Think Local Act Personal (TLAP) describes some outcomes and success factors in the care and support of children and young people with complex lives, from their perspective and that of the people who are important to them.
